Appointment reminder software: how to reduce no-shows with texts that follow the rules

By Ranjit Bhinge, founder of Blur Studio · Updated · 7 min read

Short answer: Appointment reminder software sends automatic texts, emails or calls before each booking and lets clients confirm, cancel or rebook with a reply. A Cochrane review found that text reminders raised attendance compared with no reminder. As of October 2026, Acuity Scheduling with text reminders starts at $27 a month billed yearly, Apptoto starts at $30 a month and Square Appointments lists reminders on its free plan. Under the TCPA, get consent for reminder texts and keep promotions out of them.

Every no-show is a slot you cannot sell again. For a salon, a clinic or a tradesperson, a few each week add up to real money. Automatic reminders are one of the cheapest fixes, bec

I'm Ranjit, and I run Blur Studio: 15+ years building software, 120 apps taken live, and clients who have gone on to raise $10M+. I build AI systems and processes that help businesses do a lot more with a lot less time. I also work differently from most of what else is out there, more on that further down this page. Say hello.

Do appointment reminders really reduce no-shows?

Yes, the research supports it. A Cochrane review looked at seven studies with 5,841 people in healthcare settings. It found that text reminders raised attendance compared with no reminder, with a risk ratio of 1.14. In plain words, people who got a text were more likely to show up.

The same review compared texts with phone call reminders in three studies. Texts worked about as well as calls, and two of those studies found that texts cost less. That is why most businesses now send texts first.

What does appointment reminder software do?

  • Automatic reminders: The tool sends a text, email or voice call at set times before each booking.
  • Two-way replies: Clients reply "C" to confirm or ask to cancel or move the time.
  • Calendar sync: Reminders pull from your booking calendar, so you never type them by hand.
  • Waitlist offers: When someone cancels, the tool can offer the open slot to the next person.
  • Deposits and card on file: Some tools charge a fee for a late cancel or a no-show.
  • Opt-out handling: The tool stops texts when a client replies "stop" and keeps a record of it.
  • Reports: You see confirm rates and no-show rates by staff member or service.

What does appointment reminder software do?

Turning on two-way replies without a plan for what happens next is a common and costly mistake. A client who texts back "can we move this to Friday?" instead of a plain confirmation needs a real person to see that message and act on it. If those replies land in an inbox nobody checks, you end up worse off than before: the client believes they rescheduled, your calendar still shows the old slot, and you get a no-show you could have avoided entirely. Route anything other than a clear yes or no to a person, not just a report.

Which appointment reminder software should you use?

Your booking system may already send reminders. Check that first, since a separate tool adds cost and another login. Salon, barber and job software often includes them.

Tool Starting price Text reminders Good fit
Square Appointments Free plan Reminders listed on the free plan Salons and small shops that also take payments
Acuity Scheduling Standard $27 a month billed yearly, or $34 monthly On Standard and Premium Coaches, consultants and clinics booking online
Apptoto From $30 a month, plus $10 per extra user Texts, emails and voice calls Businesses that keep their current calendar

Square Appointments

Square's free plan lists appointment reminders, and Square says its reminders can go out by text and email. Its pricing page also lists Square Assistant, which lets clients reply to a reminder text to confirm, cancel or change the booking. Cancellation policies and no-show fees start on the Plus tier. Its fee table shows Square Plus at $49 a month and Premium at $149 a month per location.

Acuity Scheduling

Acuity's Starter plan costs $16 a month billed yearly or $20 monthly, but it does not include text reminders. Standard costs $27 a month billed yearly or $34 monthly and adds text reminders. Premium costs $55 a month billed yearly or $69 monthly. Premium is the only plan where Acuity will sign a business associate agreement (BAA) for HIPAA.

Apptoto

Apptoto works on top of the calendar you already use, such as Google or Outlook. It starts at $30 a month, plus $10 for each extra user, and yearly billing takes 23% off. It sends reminders by text, email or voice, with two-way replies. One credit covers a text of up to 300 characters, a call of up to two minutes or an email. Its HIPAA plan, with a signed BAA, starts at $208 a month and needs at least 16 calendars.

How do you cut salon no-shows?

Salons lose the most when a long color or extension job does not show up. Reminders help, but they work best with a few other habits.

  • Send two reminders: One a day or two before and one a few hours before works for most salons.
  • Ask for a reply: A reminder that asks the client to confirm gets more attention than one that only informs.
  • Take a deposit on long services: A card on file or a deposit gives clients a reason to cancel early.
  • Run a waitlist: Offer a cancelled slot to clients who asked for an earlier time.
  • Track repeat no-shows: Ask clients who miss twice to pay up front next time.

The TCPA is the federal law on robocalls and automated texts. The FCC's rules under it cover text messages as well as calls. The level of consent you need depends on what the text says.

  • Reminder texts: For automated calls and texts to a mobile number, the rules need the person's "prior express consent". The simplest proof is a line on your booking form that says you will text reminders. Keep a record of when and how each client agreed.
  • Marketing texts: Texts that advertise or sell need "prior express written consent". That is a signed agreement, which can be electronic, that names the phone number. It must say the person does not have to agree in order to buy.
  • Mixed texts: A reminder with a coupon or a sales offer can become a marketing text. Keep reminders to the time, place and how to change the booking.
  • Opting out: People can withdraw consent in any reasonable way, such as replying "stop". You cannot force them to use only one method. You must honor the request within 10 business days.

Healthcare reminders

The FCC rules have a narrow exemption for texts and calls from healthcare providers. It covers messages such as appointment reminders, lab results and prescription notices, with strict limits.

  • Patient's own number: The text goes only to the mobile number the patient gave you.
  • Free to the patient: The message must not cost the patient anything or count against their plan.
  • Short and plain: Texts must be 160 characters or less, name the provider and give contact details.
  • Limited count: One message a day per patient, and no more than three a week.
  • Easy opt-out: Each message must offer a simple way to stop, and you must honor it right away.

These texts must not contain marketing, sales or billing content, and they must follow HIPAA privacy rules. If patient data passes through a reminder tool, ask the vendor to sign a BAA. Acuity and Apptoto both offer one on certain plans. My dental practice management software guide covers tools built for clinics.

Carrier registration

US carriers also require A2P 10DLC registration for business texts sent by software from a normal 10-digit number. Apptoto lists 10DLC compliance on its plans. My guide to missed call text back explains how registration works and what your provider needs from you.

How do you set up reminders that work?

  1. Get consent at booking: Add a line on your booking form saying you will send reminder texts, with a way to opt out.
  2. Write short, clear texts: Include your business name, the date and time, the address and how to reply.
  3. Pick your timing: Start with one reminder 24 to 48 hours before and one a few hours before.
  4. Turn on two-way replies: Let clients confirm or cancel by text, and route other replies to a person.
  5. Connect your waitlist: Fill cancelled slots right away.
  6. Check your numbers each month: Compare no-shows before and after, by service and staff member.

When does a custom reminder system make sense?

Most businesses should use the reminders built into their booking or job software. A custom setup can make sense in a few cases.

  • Your bookings live in several systems: For example, a clinic with one system for therapy and another for classes.
  • You need smart follow-up: Such as an AI agent that reads a reply like "can we do Friday?" and moves the booking itself.
  • You have strict data rules: Some clinics and law firms need full control over where patient or client data goes.
  • Message volume is high: At large volumes, your own system on a texting provider may cost less than per-user plans.

How would Blur Studio approach appointment reminders?

I would start with your no-show rate and where your bookings live. If your current software already has good reminders, I will tell you to switch them on and tune them. If you need more, I connect your booking systems to one reminder flow. I can add an AI agent that handles replies and rebooks clients, with consent and opt-out records built in. Your business owns the code and accounts, and I keep it running after launch.

Questions people ask

What is the best appointment reminder software for a small business?

Start with the reminders in your booking software, since many include them. As of October 2026, Square Appointments lists reminders on its free plan, and Acuity Scheduling adds them from $27 a month billed yearly. Apptoto suits businesses that want reminders on top of an existing calendar.

Do I need consent to send appointment reminder texts?

Yes, the TCPA rules require prior express consent for automated texts to mobile numbers. Tell clients at booking that you will text reminders and keep a record. Marketing texts need prior express written consent.

How far ahead should you send appointment reminders?

Many businesses send one reminder 24 to 48 hours before and a second a few hours before. Test your timing and compare no-show rates each month.

Can a reminder text include a promotion?

Adding a sales offer can turn a reminder into a marketing text, which needs prior express written consent. Keep reminders to the booking details. Send offers separately, only to people who agreed to marketing texts.

Do text reminders reduce no-shows?

Yes, a Cochrane review of seven studies found text reminders raised attendance at healthcare appointments compared with no reminder. Texts worked about as well as phone calls and cost less.

What happens if a client replies to a reminder text asking to reschedule?

That reply needs to reach a real person, not just sit in a report nobody reads. A common and costly mistake is turning on two-way replies without a clear path for anything beyond a plain yes or no, which can leave a client believing they rescheduled while your calendar still shows the old slot.

Roughly what does this cost?

As a broad reference, fixes and finishes start at around $1,500, and new builds start higher, always a fixed price agreed in writing before I start, never an hourly rate. My AI app cost guide explains what actually drives a quote up or down.

Is my project too small for this?

No. Most of the owners I work with have no tech team at all, that is the whole point. If the smarter move is a cheap software subscription instead of custom software, I will tell you that on the first call, even though it means I do not get paid.
